New Client Forms
Required paperwork for all clients:
Please print, read, and sign the Counseling Practices and Procedures and Confidentiality Agreement forms, and bring them with you to your first session. Or you may also upload them below if you prefer.
Additional required paperwork for adolescents:
In the first session with an adolescent under 18 years of age, both a parent and the adolescent will sign the form together with the therapist. It is not necessary for both parents to sign and attend the first session, but it is preferrable when possible.
